A 60-minute virtual session where Ciara gets to know you, reviews your history, and begins building your personalized care plan.

Who You Are

Your name, age, and whether you're reaching out for yourself or on behalf of a child or family member. Parents and guardians are always welcome to contact us on behalf of a minor.

What You're Experiencing

A brief description of what's been going on — even just a sentence or two. You don't need to write a medical history. "I've been struggling with anxiety and can't sleep" is more than enough.

Your Insurance or Payment Plan

Let us know if you have insurance (and which plan) or if you're planning to self-pay. This helps us verify your benefits before your first appointment so there are no surprises.
